I’ve quoted your original post here since we generally like to leave solved problems up so that other users can see the problem and solution. We do not recommend using this driver with a 16V supply since 16V is the absolute maximum voltage the driver will tolerate and brushed DC motors can generate voltage spike in many situations (which could cause the driver to fail in a short and burn). Also, you should use an electrolytic capacitor on the motor supply close to the board to help limit the size of voltage spikes and to prevent LC spikes from damaging the board.
