Hi. I would like to use the S18V20AHV in a prototype for use in a car. What would you recommend, if anything (beyond a fuse), to protect the regulator when powered by the 12v supply in a car?

The automotive environment can be very harsh, and it is possible to get transients higher than the 30 V maximum of that regulator. If you want to be safe for sure, you can look at documents such as this Littelfuse application note for information about how to deal with those transients.
